What forms do I need to complete ?

  • First time users of the Global Plant Clinic need to complete and submit the user login form. This form enables us to keep a track of who is making use of this service and is required before submission of samples to the clinic, and before an advisory request is made.
  • Once you have received confirmation of your login name and password please follow the appropriate links to the sample submission form or advisory request form.
  • Before submitting a sample, you must have completed the online sample submission form. If you have problems with this form, it is also available by request as a Word document, by contacting the Global Plant Clinic. Please fill in all fields.
  • When submitting a sample you must also print out and complete section 1 and (depending on local regulations) section 2 of the relevant import licence. Failure to include the import licence with your samples may result in your samples being destroyed by Customs. Please be aware that each country has its own local regulations for sending material so check with your local authorities or Post Office before sending.

General Import Licence

Covers all plant and soil material infected with fungi and bacteria.

Nematode Import Licence

Covers the import of all material with suspected nematode infection.

Viral import Licence

Covers the import of all material with suspected viral infection.

  • Finally, if you have not already done so, please read the relevant information on collecting, packaging and sending material to the clinic. Samples that arrive at the clinic in a poor condition are difficult, or impossible to diagnose.
